Your PIN, or Provider Identification Number, is a unique number assigned by PPNI to each provider. If you received a post card reminder, the PIN will be displayed at the top of the mailing label. Your PIN can also be found on your contract, or by calling our Provider Relations department at (866) 776-4872.
